Acute Aortic Dissection Review Articles

Acute aortic type A dissection (ATAAD) is still one of the most difficult diseases that cardiac surgeons encounter. This review is based on current literature and includes the results of the NORCAAD (Nordic Consortium for Acute Type-Aortic Dissection) database. It covers different aspects of ATAAD and focuses on the results of surgical repair. Diagnosis is sometimes delayed and ATAAD is generally fatal if prompt repair is not performed. The dynamic nature of the disease, the variation in presentation and clinical course and the urgency of treatment require great attention. Numerous surgical techniques and perfusion strategies of varying complexity have been described, ranging from simple transplantation grafting to total replacement of the arch of the foot with a frozen elephant's trunk and root reconstruction sparing the valve. Although more complex techniques may offer long-term benefits for some patients, they require significant surgical expertise and experience. Short-term survival is the first priority, so an accelerated operation that matches the level of expertise of the surgeon is in most cases appropriate
