Acute aortic dissection is defined as dissection occurring within 2 weeks of the onset of pain. Subacutehttps://www.ncbi.nlm.nih.gov/pmc/articles/PMC5831732/ and chronic dissections occur between 2 and 6 weeks, and more than 6 weeks since onset of pain, respectively
Two classifications are most commonly used for aortic dissection (Fig. 1). The DeBakey system is classified into three types (types I, II and III) according to the site of the first dissection entry. Type I first enters the ascending aorta and spreads distally to the descending aorta. Type II has the first entry into the ascending aorta and does not spread to the aortic arch. Type III first enters the descending aorta and spreads distally above (type IIIa) or below (type IIIb) of the diaphragm. The Stanford system is classified into two types (types A and B) according to the involvement of the ascending aorta. Type A includes dissection in the ascending aorta regardless of the site of first entry. Type B does not include dissection in the ascending ort.
Continued advances in imaging and treatment technologies improve short and long-term outcomes in patients with acute aortic syndrome. Knowledge and interest in intensive care medicine in this area contributes to improved outcomes, and further research into this life-threatening disease will improve diagnosis and management.
