Abstract :

Acne scarring and Papulopustular Rosacea (PPR) are well documented cutaneous condition associated with major psychosocial morbidity: Social isolation, depression and suicidal ideation. The negative aesthetic consequences of poorly managed acne are significant to an individual quality of life and career. A positive family history is a predictor of scar formation. Emerging data revealed early retinoids used, positive life-style modifications may prevent scarring. Energy device like radiofrequency microneedling, fractional laser, superficial radiotherapy and intralesional cryotherapy are recognized new attempts to treat acne scars and painful severe enlarging keloid scars in Asian dark skin types, respectively. Low dose systemic isotretinoin was shown efficacious in managing teenage and adult onset acne vulgaris; including acne scar prevention. More and more data are emerging on low dose retinoid therapy both topicals and systemic has add-on aesthetic and rejuvenating effects on acne prone facial skin. The text book paradigm that systemic retinoids are contraindicated in energy device procedure will be challenged; clearly no major adverse effects seen in this combination therapy in Asian skin types. The lecture will discuss these new strategies and examined its potential advantages and limitations. PPR are increasing in prevalence as seen in local skin clinics due to increasing non-judicious topical steroids use; propensity of topical cosmeceuticals in Asian markets and the use of Epidermal Growth Factor Receptor (EGFR) inhibitors in treating metastatic lung cancer. Recent investigations highlighted the importance of the demodex mites, Vascular Epidermal Growth Factor (VEGF), dermal inflammation and impaired epidermal barrier dysfunction in its pathogenesis. New promising treatments are emerging.
