Vertebral compression fractures affect at least a quarter of all postmenopausal women. The most important risk factor is osteoporosis, most often seen in Caucasian women some ten years after menopause. Osteoporosis usually results from an inadequate build-up of bone mass during childhood and early adulthood, followed by rapid absorption after menopause. Primary treatment for osteoporosis includes taking into account underlying metabolic abnormalities and providing calcium / vitamin D supplements in combination with bisphosphonates or calcitonin, or both. Routine hormone replacement therapy has fallen out of favor due to concerns about the side effects identified in long-term follow-up studies. Managing vertebral fracture by osteoporotic vertebral compression includes strengthening, pain relievers, and functional restoration. Patients with chronic pain beyond 2 months may be suitable candidates for augmentation of the vertebral body, i.e. vertebroplasty or reduction of balloon compression. Open surgical management with decompression and stabilization must be reserved for the rare patient with neural compression and progressive deformation with neurological deficits.
Osteoporotic spinal compression fractures are a common complaint for spine care specialists. A thorough differential diagnosis should be considered before attributing fractures to osteoporosis. Appropriate assessment and medical treatment of the underlying osteoporosis should be recommended or instituted. Non-surgical management of the vertebral fracture should focus on controlling pain and maximizing functional outcomes. The role of surgical treatment remains controversial and should be reserved for patients who fail the first options of non-surgical management.
