Introduction

Polychronic conditions, also known as multi-morbidity or multiple chronic conditions, refer to the coexistence of two or more chronic health conditions in an individual. These conditions pose significant challenges for both patients and healthcare providers, as they often require complex management strategies and can have a substantial impact on the patient's quality of life. In recent years, predictive analytics has emerged as a valuable tool for improving the care and outcomes of patients with polychronic conditions.
