In the eight Millennium Development Goals, the World Health Organization has proposed improving the epidemiological rates of HIV / AIDS worldwide through the prevention and treatment of disease as a goal to be achieved in 2015. Achieving the one of these objectives is the virtual global eradication of vertical transmission (prevention of mother to child).
This chapter will cover topics related primarily to diagnostic and therapeutic aspects that have proven useful in preventing vertical transmission of HIV (TV). The objective is to provide the reader with a clinical guideline for the management of an HIV-positive pregnant woman based on the levels of evidence, the scoring of recommendations and expert opinions and to individualize health care strategies which have been shown to be effective in reducing vertical transmission of HIV. Such an achievement will highlight the importance of ordering an HIV test from all pregnant women during the prenatal check-up. The latter, in case of carrier status, will help reduce the viral load of HIV (+) mothers to undetectable or almost undetectable levels. It will also allow the formulation of a management strategy based on clinical and laboratory parameters, with the use of the most adequate pharmacological management, thereby reducing the exposure of the newborn to an HIV mother. (+), blood, genital secretions or amniotic fluid and to identify the best opportunity to schedule the delivery and cessation of breastfeeding.
This clinical guideline is intended for midwives, medical students, fellows / residents in gynecology and obstetrics, fellows / residents in maternal and fetal medicine, obstetricians working in level 2 and 3 hospitals, and medical specialists. maternal and fetal medicine, which aim to update their knowledge on the diagnosis and management of pregnancy affected by HIV / AIDS.
