Researchers found no participant-to-participant transmission of HIV among nearly 900 HIV-positive couples who continued to report having sex without a condom while the HIV-positive partner was taking suppressive ART.

The participant prospective and observational study was conducted with heterosexual couples and men who have sex with men in Europe and included a median follow-up of 1.3 years.

"Although these results cannot directly answer the question of whether it is safe for [serodiscordant] couples to practice condom less sex, this study provides informative data (especially for heterosexuals) for that couples base their personal acceptability of risk on serodiscordant couples. (n = 888) from 75 clinical sites in 14 European countries who reported having sex without a condom while the partner infected with ART had an HIV-1 RNA viral load of less than 200 copies / ml.

From September 2010 to May 2014, couples (61.7% heterosexual) reported 1,238 couples-years of follow-up. During this period, couples reported having sex without a condom a median of 37 times a year. The total number of sex acts without a condom reported by MSM couples was around 22,000. For heterosexual couples, it was about 36,000. Among MSM couples, 108 HIV-negative partners reported having sex without a condom outside of the relationship, while 21 HIV-negative partners in heterosexual couples did the same.
