Heart Regeneration Innovations

Ischemic heart disease is the leading cause of death worldwide. Blockage of the coronary arteries limits oxygen-rich blood to the heart and, as a result, there is the death of cardiomyocyte  (CM) cells, inflammation, fibrotic scars and myocardial remodeling. Unfortunately, current therapies fail to effectively replace lost cardiomyocytes or prevent fibrotic scarring, which results in reduced heart function and the development of heart failure (HF) in the hearts of adult mammals. In contrast, neonatal mice are able to regenerate their hearts after an injury. However, this regenerative response is limited to the first week of postnatal development. Recently, we have identified that signaling of the cholinergic nerve is necessary for the mouse neonatal regenerative response. This demonstrates that stimulation of the cholinergic nerve holds significant potential as a bioelectronic therapeutic tool for heart disease. However, the mechanisms of nerve regeneration directed in the heart remain undetermined. In this review, we will describe the historical evidence for nerve function during regeneration between species. More specifically, we will focus on the emerging role of cholinergic innervation in modulating the proliferation and inflammation of cardiomyocytes during cardiac regeneration. Understanding the role of nerves in mammalian heart regeneration and cardiac remodeling in adults can provide us with innovative therapeutic approaches based on bioelectronics for the treatment of human heart disease.
