Abstract :

For over 23 years, the Liver Transplantation Nursing Team at Kaohsiung Chang-Gung Memorial Hospital has been a pioneering force in living donor liver transplantation (LDLT) care. In Taiwan, our "Liver Transplantation Care" was awarded the "National Biomedical Quality Award" in 2014. The nursing team performs a range of responsibilities with the ultimate mission of providing high quality patient-centered care. These include comprehensive pre-operative assessment, psychological and social support for patients and families, intraoperative support, and the full range of postoperative care. We have been responsible for innovations in preoperative instructions, intraoeprative safety improvement during surgery and postoperative care. The preoperative instructions educate the patients about surgical procedures to lesson anxiety of patients and drug compliance to increase the accuracy of medication. During operation, we design a comprehensive protection for positioning the donor and recipients to avoid hypothemia and peripheral neuropathy from prolonged surgery. In ICU, total nursing care without contacting the patients and family, real-time inform system of dangeous values of examinations, bundle care for catheters to minimize the risks of infection will be taken. Postoperative care in the ward specializes in the customized instruction guideline for medications, nutritional support system. Prioritizing superb pre-operative, intra-operative, and posttransplant care has contributed to excellent patient outcomes at our center that are among the best in the world. Under the joint efforts of the liver transplantation nursing, surgical, and medical teams, KCGMH has achieved the highest LDLT volumes in Taiwan, and our patients have a one-year graft survival rate of 95.5% and five-year survival rate of 90.5%, which are the best in Taiwan and superior to LDLT outcomes in the world.
