Severity and injury characteristics among matched hospitalized motorcycle drivers and their passengers in a trauma center of Shiraz, Iran

Abstract :

After car accident, motor cycle accident is the most cause of fatality in Iran. This study aimed to compare the severity and clinical presentations between matched couples of driver and passengers under the same injury circumstances. This study was done in the trauma center of Shiraz, Iran from the 1st of January to 30th of June, 2017. Data on demographics, BMI, helmet use, having a driver's license, triage level, blood pressure, respiratory rate, GCS, injured body region, ISS, RTS, and death were compared between pairs of drivers and passengers. This study included 143 matched pairs of drivers and passengers. There was no significant difference between the drivers and passengers according to survival; RTS; blood pressure; BMI; triage score; and injury to the head and neck, thorax, face and soft tissue skin injury. ISS was significantly more in drivers than passengers. Drivers were found to suffer more injury in the abdomen, extremities, pelvis and spine. In the matched pairs, drivers and passenger did not show any difference in sustaining injuries in the face, head neck, chest and soft tissue skin but once abdomen and extremity region of the one matched pairs was injured, the chance of damage in other matched pair was 6.8% and 6.3%, respectively which is significant. Although drivers had higher levels of injury severity and some injury distribution treatment, management and equity towards drivers and passengers were equal. We also recommend more police supervision for helmet use, driving license and third-party insurance.
