Mucinous Cystadenoma of the Ovary with Xanthogranulomatous Oophoritis: A Rare Pathological Association

Abstract :

Statement of the Problem: The coexistence of mucinous cystadenoma of the ovary and xanthogranulomatous oophoritis represents an unprecedented pathological association in the medical literature. While each condition has been reported individually, their simultaneous occurrence within the same ovarian tissue has not been previously documented. This unique presentation poses diagnostic and therapeutic challenges, underscoring the importance of increased clinical awareness and interdisciplinary management. Methodology & Theoretical Orientation: A 25-year-old woman presented with abdominal pain, fever, and a palpable left adnexal mass. Despite antibiotic treatment for presumed pelvic inflammatory disease, symptoms persisted. Ultrasound and transvaginal imaging revealed a 9 × 6 cm thick-walled, multilocular ovarian cyst. Exploratory laparotomy confirmed a mucinous cystadenoma with adjacent xanthogranulomatous inflammation. Histopathological examination demonstrated mucinous epithelium, subepithelial fibrosis, chronic inflammatory infiltrates, and foamy macrophages. Findings: This case highlights the interaction between chronic inflammation and benign neoplasms, suggesting that localized inflammation may influence the pathogenesis of such rare associations. Postoperatively, the patient recovered without recurrence. Conclusion & Significance: This is the first documented case of mucinous cystadenoma coexisting with xanthogranulomatous oophoritis. It emphasizes the need to consider rare pathological associations in patients with complex ovarian masses. A multidisciplinary approach is crucial for accurate diagnosis and successful management. Future research should investigate the mechanisms linking inflammation and ovarian neoplasms to refine diagnostic and therapeutic strategies.
