Introduction: Chronic Renal Insufficiency (CRI) represents the process of progressive reduction of the ability of the kidneys to perform their functions, which is followed by the disorder of blood pressure, vitamin and hormone metabolism, and blood clotting. Purpose: We presented a 63-year-old male patient with main complaints such as lethargy, fatigue, nausea, consciousness disorders, frequent urination. Material and method: It comes from the emergency of the regional hospital, where after the laboratory analysis, it results in increased values of nitrogen substances in the blood and hyperkalemia. The analyzes on admission were; urea- 36, creatinine- 443, SE- 110/, CRP-71, ER- 2.85, HB-69, HCT- 25. Results: Based on the clinical condition and laboratory analyses, a hemodialysis session is indicated, the central venous catheter is placed in the right femoral vein and dialysis is performed without complications. In the urine culture it is isolated: E.coli sensitive to imipenem. On CT of the abdomen, bilateral renal stasis, expansive process in the ampulla recti. Conclusion: The patient with exacerbation of chronic kidney disease, with the presence of primary malignant disease (tu recti), with apparent arterial hypertension, as well as a state of uremia with metabolic acidosis. 7 hemodialysis sessions were performed, 4 doses of deplatated erythrocytes were prescribed. From the result of the ultrasound of the urotract, it was established that we are dealing with grade IV hydronephrosis, the urologist was consulted, who indicated and carried out the placement of the nephrostomy, which was functional, now the values of nitrogenous substances marked a satisfactory decrease (creatinine 377umol/l), since hemodialysis sessions are stopped. The patient has been monitored for several days, after improving the clinical and laboratory conditions, he was discharged home in a stable general condition. To continue treatment by the oncologist and urologist.
