Abstract :

This mutual collaborative action research aimed to develop the fast track service and referral system for patients with STEMI. Target groups consisted of four groups: (1) Data record forms for STEMI patients, (2) STEMI patients, (3) Caregivers and (4) Nursing case management. The study was conducted from February to July of 2016. The study process comprised of three phases: Situational analysis, development and evaluation. The instruments included a data record form for acute myocardial infarction, an evaluation form for practice following a standard care pathway, an evaluation form for nursing practice following a clinical nursing practice guideline, a questionnaire for interviewing caregivers and a questionnaire for focus groups. Descriptive statistics and content analysis were used to analyze quantitative data and qualitative data. Situational Analysis Phase: Problems with fast track service were reported in both process and outcome domains. Development Phase: Three cycles were conducted in development processes: Rapid access to acute myocardial infarction fast track, patient screening, care process by multidisciplinary team members, consultation system, development of a standard care pathway, a clinical nursing practice guideline and tools of referral and patient follow up and outcome evaluation. Evaluation Phase: 100% of nurses practiced following a clinical nursing practice guideline. Average time from door to EKG was 15 to 5.22 minutes. For door to consult EKG was 20 to 8.22 minutes. The patients received streptokinase within 29 minutes and all of them were referred after receiving streptokinase.
