Application of skin care bundle in decreasing prevalence of pressure injury in intensive care unit

Abstract :

Pressure injury, which is an important care issue, is an indicator for quality care in western countries. The Joint Commission of Taiwan added pressure injury as a critical indicator for nursing quality care in 2011. Not only prolongs hospitalization, pressure injury increases medical costs and infection rate and therefore threatens patientsâ?? lives. Prevention of pressure injury is especially an important nursing care issue in intensive care unit. From January and April, 2018, prevalence of pressure injury was 51% in the intensive care unit and the rate was higher than it (20%) in other units in our hospital. We therefore designed the skin care bundle by applying simple and routine nursing interventions to decrease prevalence of pressure injury in the intensive care unit. We reviewed medical charts and observed/audited nursesâ?? skin care skills to analyze factors of pressure injury. Combined with literature reviews, we established a skin care bundle including skin safety collaboration, accurate report, hospital goal, education, skin examination/assessment, constant pressure relief, incontinence prevention, and nutrition assessment. After 7 months implementation of the skin care bundle, prevalence of pressure injury decreased significantly from 51% to 22%. Pressure injury is often caused by multiple factors. Bundle care, which makes preventive interventions complete, can prevent occurrence of pressure injury and improve nursing care quality.

Biography :

Hui-Na Tsai has been a Clinical Instructor for more than 10 years and has been a Registered Nurse for more than 15 years. She is currently a Nurse Manager in an intensive care unit. She has obtained her Master's degree in Nursing. She had published several articles and participated in many conferences globally.
