The clinical evaluation of patients with thyroid nodules is a common problem facing the clinician. The vast majority of these nodules are benign, but the concern that such a thyroid swelling may harbor a malignant tumor requires a rapid and precise diagnosis. In addition, it is clear that a properly treated differentiated thyroid carcinoma is associated with an excellent prognosis. The goal of studying patients with thyroid nodules is to define the small number of malignant tumors with minimal inconvenience to the patient in the most cost-effective way. There are no laboratory tests that reliably differentiate benign from malignant disease. Traditional approaches to radionuclide and ultrasound have been shown to be sparse in diagnosing malignancy, leading to many unnecessary operations for benign lesions. These tests have been replaced in many centers by fine needle aspiration cytology, with surgery alone for abnormal cytological results. This technique is easily performed in an outpatient clinic and is well tolerated; an accuracy of the diagnosis of thyroid neoplasia of up to 97% can be obtained.
