Subcutaneous emphysema (SE) is a condition that often causes minimal symptoms, but sometimes it can be serious and even life-threatening. This study is the first major investigation of SE. The aim of this study is to classify and assess the etiology, signs, symptoms and management of ES. This retrospective study was performed by examining patients diagnosed with Rasht SE between January 2001 and January 2011. We classified the severity of SE into five degrees, including (1) the base of the neck, ( 2) all of the neck area, (3) the main sub-pectoral area, (4) the chest wall and the entire neck area, and (5) the chest wall, neck, orbit, scalp , the abdominal wall, the upper limbs and the scrotum. We excluded all grade 1 and 2 patients because the symptoms and signs were not significant. A statistical analysis was performed with SPSS 18. We collected 35 cases of ES with an average age of 53 ± 14.83 (71% of men). The most common cause of ES was pneumothoraxis with a history of COPD and surgery in grade 5, trauma due to a rib fracture in grade 4 and atiogenicity in grade 3. We made two bilateral incisions of 2 cm subclavicular. In our patients with infraclavicular incisions, the lung expansion was better and the appearance of the patients improved. Infraclavicular incisions as a simple method for the management of EM can decrease the severity of EM without cosmetic problems.
