Paget's disease of the bone is a common bone disorder characterized by disorganized bone turnover. The most likely etiology is a slow paramyxoviral infection in genetically sensitive individuals; however, the exact cause is unknown. Improved bone resorption due to increased osteoclast activity recruits many osteoblasts at the resorption sites, which produces large amounts of new bone matrix. Accelerated resorption and bone formation are not as tightly coupled as in healthy bone; disorganized bone tissue forms. Many patients are asymptomatic; the increase in serum alkaline phosphatase or the incidental discovery of characteristic radiographic lesions are often the only diagnostic clues. Common clinical manifestations include bone pain, curvature of long bones, enlargement of the skull, and hearing loss. Elevated serum alkaline phosphatase levels correlate with disease activity. The diagnosis is confirmed by characteristic radiographic results and by a nuclear bone scan (the most sensitive test). Bisphosphonates, such as zoledronic acid, pamidronate, alendronate and risedronate, are the mainstay of treatment. Patients who cannot tolerate bisphosphonates may be treated with calcitonin.
