Osteonecrosis of the femoral head is a pathological process with multifactorial origins, characterized by pain, alteration of the anatomy and biomechanics of the hip and increasing incapacity of the patient. The method of treatment depends on the stage of the disease, the size and location of the lesions, whether the involvement is uni- or bilateral, the patient's age, activity, general health conditions and his life expectancy. The unsatisfactory results obtained by saving the femoral head using the available methods have led in recent years to a renewed interest in this disease and to seek new methods and treatments aimed at stopping its development or even preventing its appearance. . We present a clinical case study of a new method involving decompression of the nucleus combined with the introduction of mesenchymal cells via a specific titanium screw called "SOIB".
