The diagnosis of musculoskeletal tuberculosis infection remains a challenge for clinicians and requires a high index of suspicion. The combination of indolent onset of symptoms, a positive tuberculin skin test and consistent radiographic findings strongly suggests the diagnosis. TB, however, must be confirmed by a positive culture or histological evidence of aspiration of synovial fluid or a biopsy of the bone or synovium. Prompt diagnosis and treatment of skeletal TB is important to prevent severe bone and joint destruction and serious neurological damage in the event of spinal damage.
