This new addition to the small number of textbooks dealing with perioperative care of the elderly patient is a large-format, multiauthored, indexed volume with many monochromatic figures and tables. It has 28 chapters distributed among seven sections labeled as General Information (Demographics and Aging Theories), Organ Reserve, Pharmacology, Preoperative Assessment, Management Controversies, Postoperative Issues, and Special Topics (Pain and Legal Considerations). There are 36 contributing authors, two thirds of whom appear to share the editor's academic affiliation. The visual presentation of the text and graphics is appealing and easy to read, and most chapters have a convenient Summary or Key Points section followed by a bibliography of reasonable length. The chapters describing mechanisms of aging, the pharmacology of inhalational and local anesthetics, critical care, and the management of acute and chronic pain are particularly informative and complete. In contrast, the chapter designated as a discussion of cardiovascular system reserve actually deals exclusively with management of myocardial infarction, cardiac failure, and other age-related cardiac diseases, an important, but much different, topic. The chapter on diastolic heart dysfunction, inexplicably placed in the Preoperative Assessment section, in fact provides the comprehensive review of age-related changes in cardiac physiology.
