Cortisol is the primary glucocorticoid that mediates the body as a stress reaction to psychological and physical strains; this response of stress is usually considered as a helpful mechanism for survival. Secretion of Cortisol may increase within minutes in conditions of acute stress, and stay at high levels for long periods, in chronic conditions. 

 

In critically ill patients, e.g. those with life-threatening diseases or sepsis, The levels of cortisol in serum, blood and urine are greatly increased (10-20 times). Thus, production of cortisol increases in conditions of stress causing deaths, which is due to increased in levels of urinary excretion of cortisol, and in many postmortem cases, High concentration levels of urinary cortisol is expected.
