Stroke is a disease with impact both wide and deep. We present three cases where we donot have consensus on the appropriate managment since they are rare. Case 1, the aortic thrombus: 60-year-old gentleman, who had a stroke with dysarthria and weakness of left side. CT revealed infarct in the cerebellum and pons and a free floating thrombus in the aorta. He was managed medically with anticoagulation with a NOAC.In our 8 patients with aortic thrombus, 4 patients had stroke. 2 patients of the total 8 required surgical intervention and others were managed with anticoagulation. Case 2, the ICA thrombus: 52-year-old gentleman, with left upper limb weakness. Infarcts were identified on MRI. CT angiogram revealed atheromatous carotids with a free- floating thrombus in the right ICA. In our 11 patients with carotid thrombus, 3 patients underwent surgery, 4 patients were given anticoagulation alone. Case 3, the MCA thrombus: Case of a 50 year-old-lady who presented with history of 4 days of dysarthia who had multiple neurological deteriorations with development of aphasia and new weakness. Revascularization was attempted with STA- MCA bypass. Conclusion In these cases opinion is divided between surgery and medical management to prevent further morbidity. We discuss the points to be considered in deciding management.
