Abstract :

This action research was aimed to develop the Long-Term Care (LTC) model for dependent elderly in Maha Sarakham municipality, Maha Sarakham Province, Thailand. The target sample consisted of a group of 4 health care providers in the LTC services, a group of 6 multidisciplinary teams in Maha Sarakham Hospital, a group of 6 health care providers from Maha Sarakham municipality and a group of 57 dependent elderly. Research instruments included a questionnaire for quality of life of elderly, an evaluation form for Bathel Index, an evaluation form for satisfaction of elderly, an evaluation from risk of elderly and questions for a focused group discussion. Data collection was conducted from October 2015 to October 2016. The data was analyzed using frequency, percentage and content analysis. The results revealed that the activities of LTC services included strong mutual collaborative teamwork in the various sectors, situational of analysis of elderly, continuing training and development program for LTC providers at all levels in the LTC service, established action plans for elderly, established care plans for elderly, implement following action plans and care plans and evaluation and knowledge management. The model consisted of 5 components: Policy, host by local sector, service networks, action plan and care plan and collaborative teamwork. The outcomes of the development model found that the Bathel Index of elderly increased to 45%, quality of life of the elderly was at good level and the satisfaction in LTC of the elderly was at good level.

Chawewan Phaophan has completed her Bachelor’s degree of Nursing Science. She is a registered nurse at Primary Care Unit in Maha Sarakham Hospital, Maha Salrkham Province, Thailand. She has considerable expertise in LTC in elderly.
