Abstract :

Coronary stent infection is a rare complication associated with a high mortality rate. Coronary stent was introduced in 1987 and since then there is extensive use of percutaneous coronary intervention (PCI) and stenting,. Coronary stent infection is not easy to treat, and associated with a high mortality rate. Patients with stent infection commonly present with fever, chills, and chest pain which can be related to bacteremia during procedure or direct septic stent implantation. More than two-third of coronary stent infections are also associated with pseudo-aneurysm of coronary arteries. Few case reports of coronary artery stent infection are reported from 1987 to 2012. However, the mortality rate has been reported to be less than 50%. Majority of coronary stent infections occur within 4 weeks of stent implantation and S. aureus is the most common organism involved. Coronary stent infection poses a serious management problem as both conservative and surgical interventions carry a high morbidity and mortality risk. While success with parental antibiotics has been reported, mortality may be as high as 50% with parenteral antibiotics alone. On the other hand, surgical intervention with resection of infected stent and surrounding material is preferred. It is suggested that surgery should be considered early in cases of late onset infections and medical therapy alone in early-onset infections unless major complications are apparent on imaging or if medical therapy appears to be failing.
