Tuberculous Meningitis

Tuberculous meningitis (TBM) develops in 2 stages. Mycobacterium tuberculosis bacilli enter the host by inhalation of droplets. The localized infection intensifies in the lungs, with spread to the regional lymph nodes. In people who develop TBM, from bacilli seeds to the meninges or cerebral parenchyma, resulting in the formation of small sub-apical or sub-ependymal foci of metastatic caseous lesions, called rich foci. The second stage in the development of TBM is to increase the size of a rich focus until it ruptures in the subarachnoid space. The location of the expanding tuber (i.e., rich hearth) determines the type of CNS involvement.
