Tuberculous brain abscess (TB) is one of the rare forms of tuberculosis of the central nervous system. A case of tuberculous cerebellar abscess in a young adult immunocompetent adult is described. There were no clinical or radiological signs of active tuberculosis elsewhere in the body. The diagnosis of tuberculous abscess was confirmed by the demonstration of acid-fast bacilli in the abscess material aspirated via a burr hole. A high index of suspicion and prompt intervention are necessary to diagnose and treat this potentially fatal but easily treatable condition.
