Tuberculous vertebral arachnoiditis involving the cauda equina is rare. A patient with lumbar tuberculous arachnoiditis in the absence of vertebral and subarachnoid tuberculosis, who mimicked a vertebral intradural extramedullary tumor is described here. The diagnosis was established on the basis of intraoperative results and was confirmed by histopathology. Surgical decompression and a combination of steroids and anti-tuberculosis therapy have worked well. At 3 months of follow-up, the patient regained control of the bladder and was able to walk with support. Clinical features, magnetic resonance imaging and intraoperative results are described. Pathology and relevant literature are discussed. Based on the patient's clinical and radiological findings, it was estimated that the patient had a caudal conical tumor and was operated on. Histological examination of the mass revealed a tuberculoma. Surgical decompression followed by anti-tuberculosis drugs has given good results.
