Spinal subdural hematoma is a rare disease with many etiologies. It is even more rare in the postoperative period and must be distinguished from the most common causes of postoperative pain and radiculopathy. We report a 36-year-old man presenting 6 weeks after surgery with debilitating pain that was found to have a collection of subdural but extra-arachnoid fluid. We describe the clinical presentation, radiological results, etiology and treatment. We conclude that, although rare, subdural hematoma of the spine should be considered in patients with postoperative pain. Rapid recognition and surgical evacuation are important for optimal recovery.
A subdural hematoma of the spine should be considered in patients on anticoagulation therapy with spontaneous signs of acute compression of the spinal cord or equine cauda. Magnetic resonance imaging with sagittal T1 and T2 images was adequate and reliable for the diagnosis of spinal subdural hematoma. Based on previous studies and the authors' intraoperative results, subdural hematomas of the spine could be considered as hematomas of the dural border of the spine. The level of preoperative neurological deficit appeared to be critical for recovery despite rapid surgical evacuation.
