Paget's Extra-Breast Disease (EMPD) is a rare intraepithelial malignant tumor that appears in areas rich in apocrine glands, such as the perineum, vulva, armpit, scrotum, and penis. We describe the case of a man in his fifties who initially presented with a small eczematous lesion on the right groin, treated with topical ointments for eczema, until an excisional biopsy of the lesion unequivocally reveals an invasive EMPD. Despite aggressive surgical procedures, her illness progressed to affect the scrotum and the penis. Deemed unresectable, the patient was treated with systemic chemotherapy with minimal response. The rarity of EMPD, in particular of the penis and scrotum, guarantees an alert eye and an increased index of suspicion when it comes to eczematous lesions of the groin in any person. An early biopsy and histological examination are crucial for early surgery of the lesions. There are no guidelines available for treating locally advanced unresectable diseases. In addition, more studies are needed to identify the genetic defects underlying the pathogenesis of this rare disease, to help improve treatment strategies and reduce morbidity.
