An entitled community response to HIV is a process by which sex workers collectively take ownership of programs to achieve the most effective HIV results and to ahead the social and structural barriers their overall health and their human rights. Community empowerment is increasingly recognized as a key approach to combating HIV among sex workers, with an emphasis on the broad context in which the increased risk of infection occurs in These persons. However, large-scale implementation of community empowerment approaches has been rare. We undertook a comprehensive review of community empowerment approaches to fight HIV among sex workers. As part of the results, we conducted a systematic review and meta-analysis of the effectiveness of community empowerment among sex workers in low- and middle-income countries. We found that community-based, empowerment-based approaches to HIV among sex workers were significantly associated with less HIV and other sexually transmitted infections, and increased use consistent use of condoms with all clients. Despite the promise of a community empowerment approach, we have identified formidable structural barriers to implementation and scaling up at different levels. These hurdles include regressive international discourse and funding constraints; national laws criminalizing sex work; and the intersection of social stigma, discrimination and violence. The evidence base for community empowerment of sex workers needs to be strengthened and diversified, including its role in helping access to combined HIV prevention and adoption interventions. Moreover, social and political change is needed regarding the recognition of sex work as work, both globally and locally, to encourage increased support for community empowerment responses to HIV.
