The point analysis of proteins in the extracellular bone matrix (ECM) represents a pivotal point for medical research in bone diseases such as osteoporosis. Studies in this area, historically carried out to assess bone biology, have been mainly carried out on animal samples and, to date, only a few studies on the detection of proteins in human bone are present. The challenges of the extraction and quantification protocols for bone ECM proteins are linked both to the separation of proteins from the mineral content (i.e. hydroxyapatite) and to the difficulty of avoiding denaturation of proteins during the extraction process. The aim of this work was to define the appropriate protocol (s) for the extraction of bone ECM proteins which could be applied to study normal and pathological conditions. We compared and optimized some of the most used protocols present in the literature, by modifying the protein precipitation method, the buffer used for resuspension and / or the volume of reagent used.Collectively, we were able to develop two single extract protocols with optimal recovery and ideal protein content, which can be used for detailed analysis of ECM proteins in pathological bone samples. Time-consuming multi-extract procedures, optimized in their precipitation methods, are however crucial for the precise detection of specific proteins, such as osteocalcin. In fact, the demineralization processes, commonly suggested and carried out in several protocols, could hinder an accurate detection of proteins, thus intrinsically affecting the study of a pathological bone ECM.
