Bromethalin is one of four distinct kinds of rodenticide fixings. It works by draining ATP from cerebrum and liver mitochondria by uncoupling of oxidative phosphorylation process which prompts disturbance of the capacity of sodium/potassium particle siphons in those organs causing cerebral edema with intracranial hypertension. Our case gave adjusted mental status after patient was found down in his vehicle with green staining around mouth and holding Tomcat rodent poison in one hand and a jug of Vodka then again. Persistent was intubated and admitted to ICU. Steady consideration was begun and later on was weaned off mechanical ventilation and improved through the span of his emergency clinic remain
