Postoperative blood testing in total hip replacement, is it needed?

Abstract :

Introduction: Demand for Total Hip replacement (THR) is set to rise significantly due to increase in prevalence of osteoarthritis. THR performed as a day case has been described, with patient selection being important to success as a day case. We observe postoperative blood derangement to be rare in THR and question the clinical value of these tests.

Methods: Patients undergoing elective THR were included in a retrospective observational study looking into their pre and post-operative haemoglobin (Hb) levels as well as post operative electrolytes. The Hb and electrolytes were assessed and compared against other variables including ASA grade, postoperative inpatient stay and 28 day readmission. 353 cases from 01/2014 to 07/2018 were included.

Results: Mean age at operation 70 (range 42-90) Mean pre-operative Hb 134.7g/l. Mean decrease in Hb at day 1 post op 22.3g/l. 6.4% of patients (n= 18) with an ASA of 1-2 had postoperative blood results requiring intervention compared to 17.8% of patients (n= 13) with ASA 3-4. Deranged bloods in the ASA 1-2 group tended to occur in older patients (mean 76.5). 28 day readmission rates were lower in the ASA 1-2 group.

Conclusion: In conclusion, our retrospective analysis of patients indicates that routine post operative bloods are not required for all patients groups. Patients with ASA grade 1-2, who are under the age of 75, rarely display derangement post operatively and routine testing does not alter clinical management.

Vipul Garg is working as speciality doctor in department of trauma and orthopaedics at Wrexham Maelor Hospital, Wrexham, since June 2018. Prior to this he was working in Singapore as a staff grade Registrar in Singapore General Hospital. He had worked in Singapore general hospital and Sengkang General Hospital for three years initially as clinical associate(Feb 2015–Feb 2016) then as registrar from (April 2016-May 2018). He had worked in Delhi, India at various Delhi Government based hospital as senior resident in trauma and orthopaedics. He had done a six-month fellowship Programme in Arthroplasty at Delhi under ISKSAA.
