Immature teratoma is a malignant ovarian teratoma composed of a mixture of tissues derived from all three germ layers. There is a classical association of the immature ovarian teratoma with gliomatosis peritonei, which is a rarely encountered complication that consists of mature glial nodular implants on the peritoneum and omentum, and it may also coexist with endometriosis. We present the case of a 42 years old female from Valle de los Reyes La Paz, who has been diagnosed with ovarian cancer four years ago. It was staged after the surgical treatment, as immature ovarian teratoma grade 3 with metastasis of a node by the histopathological report. After the surgery, she received 4 chemotherapy cycles with bleomycin; however, she started presenting some toxicity symptoms because of the chemotherapy such as neutropenia, gastrointestinal and haematological toxicity, arthralgias, myalgias and even neuropathic symptoms. As we have found in our research, gliomatosis peritonei is a benign condition that requires no adyuvant therapy. Although, there is a paradoxical benign behavior of malign teratomas when they are associated to gliomatosis peritonei, but this condition depends on two facts: Samples of peritoneum and omentum surface should be enough and, all the implants must be matures. If these two conditions are present, the prognostic of the patient is excellent.
