Spinal hematomas in the subdural space are much less common than those occurring in the epidural space. The presence of a true spinal subdural space is controversial in the anatomical literature, and manifestations of imagery of collections in the subdural space have generally received only passing consideration in much of the literature radiological. In a meta-analysis of 613 vertebral hematomas, only 4% were located in the subdural space. Contributing factors include anticoagulation, coagulopathy, percutaneous vertebral procedure, surgery and trauma. The combination of preexisting coagulopathy or anticoagulant therapy with trauma or subsequent intervention is particularly prone to the development of a subdural hematoma. Traumatic cases are somewhat rare, with only 10 cases reported, to our knowledge. Symptoms of subdural hematoma are similar to those of epidural hematoma, with an acute onset of spinal cord compression syndrome / cauda equina often preceded by back and radicular pain. Patients with subdural hematoma may have faster symptom progression than patients with epidural hematoma. Spinal subdural hematoma (SSDH) is a rare complication of spinal decompression. SSDH is classified as acute with sudden back pain or rapid symptom progression, subacute with> 1 week of symptom progression, or chronic with symptom progression over several months to several years. There has been only one previous case of acute SSDH, after decompression and fusion, this patient with bilateral pain and weakness of the lower limbs. There are only four reported cases of SSDH related to spine surgery, all of a subacute or chronic nature, and none included fusion. Although durotomy is a relatively well known complication of decompressive laminectomy, its prevalence varying from 1.6% to 14%, its association with a subsequent or concomitant SSDH in the context of arthrodesis is extremely rare. We present a case of acute SSDH, presenting a cauda equina after surgical decompression and reconstruction, as well as summarize the current literature on SSDH.
