Data can be dangerous, especially when it comes to drugs, and the numbers don't always tell the whole story. Consider the cytochrome P450 enzyme superfamily which metabolizes a wide range of drugs. The PharmVar database records about 200 missense variations for the quality encoding a solitary chemical, CYP2C19 - CYP2C19 is one of 13 qualities in the CYP2 family. However, only a relatively small number of these variants have been functionally validated. Although enzymes such as CYP play an important role in the breakdown and inactivation of drugs, they are also responsible for the activation of others. For example, clopidogrel is an antiplatelet drug that is bioactivated by cleavage of the precursor of the prodrug by CYP2C19. A patient with a loss of function mutation in CYP2C19 will not get "the antiplatelet effect you want to prevent intra-stent thrombosis and other catastrophic events after someone has a coronary stent," says Dan Roden, Sam Clark Professor of Experimental Therapeutics at Vanderbilt University in Nashville, Tennessee. "This is a manifestation of what I like to say: the most common side event is that the drug is not doing what you want."
