The June issue of the Journal of Hypertension begins with the hypertension guidelines prepared by the International Hypertension Society (ISH) and published simultaneously in Hypertension. Compared to guidelines issued by the European Cardiovascular Society (ESC) / European Hypertension Society (ESH) and the American College of Cardiology (ACC) / American Heart Association (AHA), the ISH guidelines are a more succinct document written in a read style. The stated objective is to address the hypertension problems which are also relevant for low-income countries, which may find it difficult to comply with the guidelines prepared for countries in which examinations using expensive equipment (e.g. assessment of organ damage) or complex treatment strategies. generally do not pose major implementation problems. The diagnostic and treatment recommendations strategies of the ISH directives reflect those issued by the European and American directives, but there are also some differences. Much attention, for example, is devoted to the need for physicians to obtain the best possible information on patients' adherence to treatment, given the major role played by poor compliance with the low rate of BP control plaguing the world. Adherence is notoriously difficult to measure, particularly in clinical practice, but, as discussed in detail in the ISH guidelines, collecting a series of data, sometimes seemingly trivial, can allow a doctor to understand closely how a patient is likely to behave after a prescription is issued.
