Gastric cancer is that the fourth leading cancer incidence within the world. It ranks second in cancer incidence for men, only behind carcinoma , and kills quite half 1,000,000 people a year consistent with Globocan 2000. The incidence and mortality rates of gastric cancer have steadily declined worldwide within the past few decades. The reasons for the decline in incidence and mortality rates are not fully understood; however, dietary and lifestyle factors may be associated with the phenomena, including better refrigeration, reduced consumption of preserved foods, increased intake of fruits and vegetables, improved living standards, and a decline within the prevalence of Helicobacter pylori infection. Most gastric carcinomas are sporadic (90%). In most countries, a gentle decline in gastric cancer (GC) mortality rates has been observed within the previous couple of decades. Helicobacter pylori is the major environmental risk factor for GC development, other risk factors including diet and smoking. Molecular pathogenesis of GC is complex. Gastric cancer (GC) is that the fifth commonest cancer within the world and therefore the third most frequent explanation for neoplasm-related deaths. Despite the gradually decreasing incidence of GC, its diagnosis remains related to one among the very best mortality rates worldwide. The GC symptoms are nonspecific, and this makes it difficult to establish an early diagnosis. Patients with locally advanced or metastatic GC who fail to possess a radical gastrectomy performed represent a big group of patients. Metastatic cancer diagnosis is immensely important since it helps to avoid aggressive and extensive surgery, also because it allows systemic therapy to be used . This chapter will concentrate on the clinical aspects of metastatic GC.
