A bone marrow biopsy (BMB), while not essential for diagnosing chronic phase chronic myeloid leukemia (CML), remains a first-line investigation into the analysis, prognosis, and follow-up of this disorder. It shows histological and topographic characteristics, proliferation patterns, detects metamorphosis and CML fibrosis and is essential in the investigation of unexplained cytopenias during therapy. Busalfan promotes myelofibrosis while hydroxyurea prevents it in a significant number of patients. In a study of 363 BMB taken sequentially before and after allogeneic bone marrow transplantation, there was a significant correlation of medullary fibrosis, CD61 + cells, and peripheral blood platelet counts with delayed hemopoietic reconstitution and leukemic relapse.
