The prospective, randomized, double-blind study to determine whether calcium and calcitriol prevent post-transplant bone loss. Thirty-eight non-diabetic patients and 26 diabetic patients without prior exposure to steroids undergoing their first kidney or kidney-pancreas transplant were randomized to calcium, calcium plus calcitriol or placebo. Bone mineral density (BMD) analyzes of the lumbar spine (LS), femoral neck (FN) and distal radius (DR) were obtained at baseline, at 6 and 12 months. At 1 year, placebo-treated patients experienced a 2% decrease in BMD at LS and DR and a 1.3% increase at FN. In contrast, patients treated with calcium and vitamin D experienced a decrease of 0.1% in LS and 2.9% and 4.8% in DR and FN respectively. Patients receiving cyclosporine had greater bone loss than those receiving tacrolimus. Our results demonstrate a small therapeutic effect of calcium and calcitriol and suggest that tacrolimus is less osteotoxic than cyclosporine. Healthy lifestyle choices like healthy eating, exercise and medication can help prevent further bone loss and reduce the risk of fractures. But lifestyle changes may not be enough if you've lost a lot of bone density, some will slow your bone loss and others may help rebuild bone.
